Oxygen

For patients requiring oxygen, Commonwealth employs licensed respiratory practitioners who are available to monitor and evaluate individual needs - meaning improved patient outcome for you. CHHC has the ability to offer home oxygen qualification studies through an independent testing facility - with the results sent directly to the ordering physician.

Apnea / OSA

Do you refer patients to a sleep lab when they complain about being excessively tired, snoring a lot, or having morning headaches? Before expensive testing, allow CHHC to administer Apnea Link testing - a $5.00, non-qualifying test - which provides physicians with indicators of risk.

Are you treating patients on CPAP or Bi-Pap? Commonwealth provides personalized service and supplies.

Power Wheelchairs

If a patient has mobility issues, Commonwealth is the local option to '1-800 companies' that fax reams of paperwork. As the treating physician, Medicare requires you to answer just nine (9) questions and have a face-to-face with your patient within 45 days of a power chair referral. Use CHHC's Assistive Technology Professional (ATP), who conducts a home assessment with the patient, completes much of the paperwork, and who will act as an advocate for funding, to aid your patient.

CHHC offers a complete inventory of home medical equipment for your patients! Plus we conduct fall/safety assessments during EVERY home visit.

Trained personnel are on-call and available for emergencies 24 hours a day, 7 days a week, 365 days a year.
